The Benalign PersonalCare and Balance benefit program is NOT insurance, not intended to replace insurance, and does not meet the minimum creditable coverage requirements under the Affordable Care Act. It provides direct access to providers that charge a zero co-pay for their services, or in the case of the acute and chronic Rx benefit program a small copay, based on the generic medication elected. It also includes the ability to cancel the monthly subscription and discontinue services. If a subscription is cancelled in the middle of the month, services will remain active until the end of the billing cycle and services will terminate after the last day of the paid billing period. There are no refunds for the remaining days in the billing cycle once the subscription has been paid. Subscribers must terminate the subscription before the next billing cycle by logging into their account.  

Certain services are provided by third party vendors. When connecting with a vendor, members should reference Benalign as the benefit program that they have enrolled in. Benalign is only offered in states where an active business license is in force. Please see disclosure on www.benalign.com for a listing of approved states.

All services are designed to be offered to adults over the age of 18 as the primary Benalign member.  Virtual care services and any resulting prescriptions or other care direction is subject to the discretion of the professional judgement of the licensed physician involved in the consult with the Benalign member.  Prescriptions will not be available for any DEA-controlled substance, non-therapeutic drugs, and certain other drugs that which may be harmful because of their potential for abuse. All mental health virtual services are available to members or dependents over the age of 18, unless a parental authorization is received. 

Lab services are not available in certain states. Details provided at time-of-service request. Pet telehealth is a consult-service only by a veterinary professional and is limited to services approved to be provided by each state licensing body. 

Benalign does not control the discounts offered through the Benalign Perks program, those discounts made available are subject to each respective network supplier and subject to change from time to time.
